$75.00 - $90.00 per hour

Location: Concord, CA - Hybrid. This is a field-based home health position serving patients in their homes throughout the surrounding territory, with documentation typically completed in the field.
Note: Candidates must have an active California Physical Therapist (PT) license. Home health experience, HCHB/Homecare Homebase, OASIS experience, BLS/CPR certification, and prior field-based patient care experience are preferred but not required.

Job Description
Provide physical therapy services to patients in their homes.
Evaluate patient mobility, strength, balance, and functional needs.
Develop individualized treatment plans focused on improving independence and quality of life.
Deliver therapeutic exercises and interventions designed to improve mobility and physical function.
Educate patients and caregivers on exercises, safety, and strategies to support continued progress.
Monitor patient outcomes and adjust treatment plans as appropriate.
Collaborate with other healthcare professionals involved in the patient's care.
Complete accurate and timely clinical documentation.
Travel throughout the assigned territory to provide patient care.
Qualifications
Active California Physical Therapist (PT) license required.
Home health Physical Therapist experience preferred.
HCHB/Homecare Homebase experience preferred.
OASIS experience preferred.
BLS or CPR certification preferred.
Experience providing field-based or in-home patient care is a plus.
