Home Health Physical Therapist - Patient-Centered Care with Unmatched Flexibility

Since 2008, Lifespan Therapy has empowered qualified clinicians to connect with homebound geriatric patients across California through our innovative app-based network. We're looking for experienced home health PTs or those looking to transition from outpatient or SNF settings. Join us and deliver patient-centered care with unmatched flexibility and independence.

Why Choose Lifespan Therapy?

* Your Autonomy, Your Schedule: You set your own hours and manage your caseload. Our app puts you in control of your work life.

* Streamlined Workflow: Our innovative app manages all the logistics-credentials, patient details, scheduling, messaging, and invoicing-so you can concentrate on your clinical work.

* Administrative Freedom: We handle all the behind-the-scenes tasks like referrals, authorizations, and DME, allowing you to dedicate your full attention to patient care.

* Efficient Documentation: Our integrated EMR system, TherapySync, makes compliant charting simple and fast, giving you more time back in your day.

* Career Growth: Receive consultative services to ensure your documentation meets the highest professional standards, including Medicare and Title 22.

What You'll Do

* Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.

* Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence.

* Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs.

* Collaborate for Optimal Outcomes: Work with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care.

What You'll Need

* Valid California Physical Therapist license

* CPR or BLS certification

* Driver's license and auto insurance

* Physical + TB test (within the last year)

* Professional liability and business license (if applicable)

Where You'll Be Working

Join our team as a clinician serving the creative and cultural heart of Los Angeles, spanning from the trendy streets of Hollywood and Silverlake to the historically rich neighborhoods of the Eastside. This role offers geographic flexibility, allowing you to focus on your preferred neighborhoods within this region or even serve adjacent areas that best fit your lifestyle. The Vibe: Experience a fast-paced, urban environment where you can grab a "celebrity-spotting" coffee in Echo Park or enjoy some of the city's best authentic Mexican food on the Eastside. The Community: You will be part of a dynamic, diverse community with deep roots. Because of the region's demographics, Spanish proficiency is a major asset, though not required. The Excitement: While you should be prepared for typical LA traffic, the area's unique character, endless entertainment, and vibrant street life make every day rewarding. We work with you to ensure your routes are as efficient as possible while you navigate the city's most iconic neighborhoods. Looking for someone covering any part of: Echo Park, Hollywood, Los Angeles, Silverlake, West Hollywood, Belvedere Gardens, Wyvernwood, Lincoln Heights, Omaha Heights, Pico Gardens & Hazard
