nAll About Home Care is a Houston-based home health care agency. We’ve been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for a Physical Therapist (PT) to help serve patients in the Upper Central North coverage area of Greater Houston, which mainly includes Conroe, Spring, The Woodlands, and surrounding areas. If you have experience in home health and are interested in covering this area, please submit your resume. We use Kinnser software to document patient care.
nQualifications:
nGraduate from a Physical Therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association and American Physical Therapy Association
nCurrently licensed (licensed physical therapist) in the state of Texas.
nTwo years experience preferred.
nAbility to exercise initiative and independent judgment.
nSpanish speaking a plus
nAbility to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care)
nResponsibilities:
nMakes the initial assessment visit and generates OASIS documentation the same day of the visit
nUnderstands and adheres to establish policies and procedures.
nProvides physician prescribed physical therapy.
nPerforms evaluation and treatment visits.
nImproves or minimize the residual physical disabilities of the patient.
nReturns the individual to optimal and productive level within the patient’s capabilities.
nParticipates with all other healthcare personnel in patient care planning.
nDirect and supervise their personnel as required.
nPerforms all skilled procedures as ordered by physician.
nConsults with physicians regarding change in treatment ,reports to physicians regarding patients progress, instructs patients and families/significant others in home programs and activities of daily living.
nParticipates in in-service programs and presents in service programs as assigned.
nParticipates in performance improvement activities as assigned.
nAttends all patient care conferences as scheduled.
nPrepares clinical and progress notes daily.
nAssists physicians with evaluating level of function.
nHelps develop the plan of care and revise as necessary.
nConsults with family and agency personnel.
