Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home therapy, helping patients regain strength and independence while collaborating with a care team.
You will assess functional capacity, develop individualized rehab plans, supervise PTAs and aides, document progress, and communicate with physicians, patients, and families. This role requires an active PT license, CPR, and prior team-based experience; 40-hour weeks, benefits, and competitive pay.
