Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their own homes. Reporting to the Clinical Manager, you will tailor therapy plans to individual goals and document progress.
You will work with a dedicated team of clinicians, provide clear instructions to aides, and coordinate with physicians to support recovery and return to everyday activities. A current PT license and CPR certification are required.
