Job description
CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to help patients regain strength and independence in the comfort of their homes. You will deliver personalized therapy, coordinate with clinicians, and supervise support staff to ensure high-quality care.
You will plan exercises, assess progress, and adjust plans with physicians, contributing to improved mobility and daily living activities for patients in the Chapmanville, WV area and surrounding regions.
