Job description
CenterWell Home Health is seeking a Licensed Physical Therapist to join our caring team. As a Home Health PT, you will plan and administer prescribed physical therapy in patients' homes to improve strength, mobility and independence.
You will assess patients, document progress, and develop rehabilitation plans aligned with the physician's Plan of Treatment. You will supervise PTAs and Home Health Aides, providing clear instructions and ensuring thorough documentation.
