Job description
CenterWell Home Health in Downers Grove, IL is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes. You will coordinate with the clinical team to deliver personalized care and track functional outcomes.
You will plan and administer PT, assess patient function, develop rehab programs, supervise PTAs and home health aides, and document progress for physician review and family updates.
