Job description
CenterWell Home Health seeks a licensed Physical Therapist to deliver home-based therapy under the Clinical Manager. You will plan and administer prescribed PT treatments in patients' homes, assess strength, and develop rehab programs aligned with physician plans.
You will supervise PTAs and Home Health Aides, document progress, and communicate with physicians, families, and the care team to optimize outcomes.
