Job description
CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to help patients regain strength and independence in the comfort of their homes. Reporting to the Clinical Manager, you will plan and implement personalized therapy programs that target functional improvement and safe mobility.
You will evaluate progress, document outcomes, coordinate with physicians and care teams, and guide PTAs and home health aides with clear care orders.
