Job description
CenterWell Home Health, a Humana company, is seeking a dedicated Home Health Physical Therapist to help patients regain strength and independence in their homes. Reporting to the Clinical Manager, you will design and deliver therapy plans focused on functional improvement and safe mobility.
You will assess patients, document progress, supervise PTAs and home health aides, and coordinate with physicians and families.
