Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to plan and administer in-home PT treatments, assess patient progress, and develop rehabilitation plans in coordination with physicians and the care team.
You will document outcomes, communicate progress to physicians and families, and supervise PTAs and Home Health Aides while ensuring compliance with care plans and regulatory requirements.
