Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes, reporting to the Clinical Manager. You will plan and administer prescribed physical therapy to attain the highest level of physical function.
The role requires a current, unrestricted PT license and CPR certification, experience collaborating with a healthcare team, and will involve documenting progress and coordinating with physicians and care teams.
