Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home therapy, assess patient function, and develop individualized rehabilitation plans. You will collaborate with physicians, nurses, and aides to maximize independence and functional gains for patients in their homes, under the supervision of the Clinical Manager.
Eligible candidates hold an active Physical Therapy license and CPR certification, with preferred experience in a home health setting.
