Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their own homes. Reporting to the Clinical Manager, you will deliver personalized PT services focused on functional improvement.
You will plan and administer PT treatment, assess strengths, develop rehabilitation plans, supervise PTAs and aides, document progress, and communicate with the care team.
