Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to plan and administer prescribed PT for patients in their homes, assess strength, and guide goals with the care team. You will develop rehab programs aligned with physician plans and provide clear instructions to PTAs and aides.
Reporting to the Clinical Manager, this role emphasizes collaboration with a multidisciplinary team to improve patient function, mobility, and independence in a home-health setting.
