Job description
Caretenders Home Health, a part of LHC Group, within the Optum family, seeks a Physical Therapist to join its home health team in Kentucky. You will perform patient assessments, develop treatment plans, and deliver interventions to improve mobility and quality of life.
You will document progress, communicate with physicians and family caregivers, and ensure care aligns with state guidelines and Plan of Care.
