Job description
Geisinger Home Health, part of a national post-acute care network, is seeking a Physical Therapist to assess and plan care for patients in their homes. The role involves evaluating mobility, endurance, and pain to craft treatment plans and monitor progress, working within state regulations and collaborating with the health care team.
You will conduct visits, document progress, and educate patients and families about the treatment plan, aiming to improve function and quality of life.
