Job description
Riverside Health System is seeking a staff Physical Therapist for home health in Gloucester, VA. The role includes evaluating patients, diagnosing PT needs, and planning care to restore mobility and reduce disability, while adhering to guidelines and quality standards.
The PT will perform OASIS assessments, revise plans with the physician and interdisciplinary team, supervise assistants, and support team training.
