Job description
UnitedHealth Group seeks a Physical Therapist in Home Health, joining Elite Home Health under the LHC Group family. You will assess patient needs, develop therapy plans, and provide interventions to improve wellbeing in patients and families across communities.
Responsibilities include evaluating functional status, implementing therapy plans, and coordinating with the health care team to ensure quality care. A valid PT license and transportation are required; CPR preferred.
