Capital Region Home Health, a part of LHC Group, seeks a Physical Therapist to perform assessments, determine treatment plans, and improve patient well-being in home health settings. You will evaluate functional status, mobility, endurance, and pain, coordinating with physicians and nurses to deliver integrated care.
The role includes documenting progress, creating plans of care within state guidelines, and engaging families about goals. A $10,000 sign-on is available for full-time hires.
