Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home therapy, reporting to the Clinical Manager. You will help patients regain strength, mobility, and independence with personalized care designed for functional improvement.
Responsibilities include planning therapy, documenting progress, supervising PTAs and aides, and coordinating with the care team. A $10K sign-on bonus is available for qualifying candidates.
