Job description
Moses Lake Assured Home Health, part of LHC Group within Optum, is seeking a Physical Therapist in Home Health for Washington. You will assess patient needs for mobility and endurance, develop a treatment plan, perform interventions to improve well-being, and evaluate progress.
Primary responsibilities include evaluating functional status, delivering therapy within scope of practice, documenting progress, and coordinating with family and care teams to meet goals.
