Job description
Home Care Plus Inc, part of the Optum family, is hiring a Physical Therapist for Home Health. You will assess patients, develop and adjust plans of care, and deliver interventions to improve mobility and pain.
Located in Greenbrier, Pocahontas and Monroe Counties, you will work with a care team to support patients in their homes. The role requires state PT licensure, CPR certification, and reliable transportation.
