Job description
UnitedHealth Group's St. Mary's Home Health is seeking a Physical Therapist to evaluate and treat patients in their home health program across Putnam, Cabell, Wayne and Lincoln Counties. This full-time role offers a $25,000 sign-on bonus and a $10,000 relocation bonus.
You will assess functional status, formulate a treatment plan, perform interventions to improve well-being, and document progress while coordinating with the care team. PRN options may be available.
