Job description
PruittHealth Corporation in York County is seeking a Physical Therapist for Home Health. You will evaluate function, design and implement interventions, and coordinate with the care team to support patient outcomes in the home setting.
Responsibilities include field visits, timely documentation (OASIS/SOC/recertifications), and collaboration with physicians. A state PT license and accredited program graduation are required; Home Health experience is a plus.
