Job description
CenterWell Home Health seeks a Home Health Physical Therapist to deliver in-home care, helping patients regain strength and independence. You will work under the Clinical Manager to personalize plans and monitor progress.
You will plan and administer PT treatments, assess progress, supervise aides, and document evaluations and discharge summaries, collaborating closely with physicians and the care team for optimal outcomes.
