Job description
Proforma Health Inc. seeks a Home Health Physical Therapist to provide skilled therapy to patients in their homes across designated Maryland areas.
Responsibilities include assessing patients, designing and implementing treatment plans, educating patients and families, and using adaptive devices to enhance mobility. The role requires at least 2 years of recent PT experience, OASIS-E proficiency, and a Maryland license if applicable.
