About the Role:
nTrust Home Care Inc is looking for a dedicated and compassionate Home Health Physical Therapy Assistant to join our growing team in Harford County. In this rewarding role, you'll make a meaningful difference by delivering hands-on rehabilitative care directly to patients in the comfort of their own homes. nResponsibilities:
nImplement physical therapy treatment plans under the supervision of a licensed Physical Therapist
nProvide in-home therapeutic exercises, gait training, and functional mobility assistance
nMonitor and document patient progress, reporting changes in condition to the supervising PT
nEducate patients and family members on home exercise programs and safety techniques
nAssist patients with activities of daily living (ADLs) to promote independence and recovery
nMaintain accurate and timely clinical documentation in compliance with home health regulations
nCoordinate care with interdisciplinary home health team members to ensure continuity of care
nRequirements:
nActive Maryland PTA license in good standing
nGraduate of an accredited Physical Therapist Assistant program
nPrevious home health or outpatient PT experience preferred
nValid driver's license and reliable transportation for home visits in the Bel Air, MD area
nStrong knowledge of rehabilitative techniques, therapeutic exercise, and patient safety
nExcellent communication, time management, and organizational skills
nCompassionate, patient-centered approach with a commitment to quality care
