We're currently looking for a Home Health Physical Therapy Assistant for a Home Health contract.Job Details:Job Title: Home Health Physical Therapy AssistantLocation: Denver, COSetting: Home HealthPay Range: Schedule: M-F 8a-5pContract Length: 13 weeksCandidate Type: Traveler or local candidates acceptedRequirements:1 year of Medicare certified Home Health experience. 1 year of HCHB EMR experience.Responsibilities:Provide one-on-one physical therapy services to patients in their homes.Complete patient assessments and document progress toward therapy goals.Develop and adjust individualized care plans based on patient needs and response to treatment.Coordinate care with families and other healthcare team members to support patient outcomes.Educate patients and caregivers on home exercise programs and safe mobility strategies.Why Choose VyTalent Solutions?
