Job description
Hartford HealthCare seeks a Primary Care Nurse to provide skilled nursing care in clients’ homes and community settings across Connecticut.
You will maintain client records, perform follow-up visits, teach patients and families, and coordinate care with physicians and the care team, Home Care Coordinators, and other providers.
Requires an Associate’s Degree or nursing diploma and CT RN license; one year of nursing experience; BSN preferred; home health experience a plus.
