Job description
Hartford HealthCare at Home is seeking a Primary Care Nurse to provide skilled nursing care to patients in home and community settings. You will collaborate with the care team to ensure high-quality, outcome-based care and support patients and families in their homes.
The role requires CT RN licensure, an associate degree or diploma in nursing, and at least one year of nursing experience; a BSN is preferred. You will participate in care planning and may supervise other home care staff.
