Home Health (Primarily) & Hospice RN Case Manager

Make a difference where it matters most—at home.

Superior Home Care & Hospice is a locally owned and operated agency that has been caring for patients and families in our community since 1996. As we celebrate 30 years of service, we continue to grow while staying committed to what matters most: providing exceptional clinical care with compassion, dignity, and respect.

We are looking for a Home Health & Hospice RN Case Manager who values relationships, enjoys working independently, takes pride in providing excellent care, and wants to be part of a supportive team where their work truly matters. This position is primarily focused on home health patients, with occasional hospice visits and support as needed.

If you are compassionate, organized, dependable, and motivated by helping people remain safely and comfortably in their own homes, we would love to meet you.

Why Join Superior Home Care & Hospice?

At Superior, you have the opportunity to build meaningful relationships with patients and families while making a difference during some of the most important moments of their lives.

Patient-centered care — You have the time and autonomy to understand what matters to each patient and family.

Meaningful relationships — Get to know your patients rather than simply moving from task to task.

Professional autonomy — Manage your own patients and use your clinical judgment while having a supportive team available when you need it.

Teamwork — Collaborate with physicians, therapists, aides, social workers, chaplains, and fellow nurses to provide comprehensive care.

Work-life balance — Our typical schedule is Monday–Friday, 8:30 a.m.–5:00 p.m., with flexibility when possible. Typical productivity is 5–6 visits per day. Territory is in the Salt Lake Valley. Caseloads are managed to support quality patient care and work-life balance.

A supportive environment — You won't be expected to figure everything out on your own. We have a strong leadership team, and training and ongoing support are provided.

What You'll Do

As an RN Case Manager, you will primarily provide skilled nursing care and care coordination for home health patients, while occasionally supporting hospice patients and families as needed.

Perform comprehensive nursing assessments and ongoing patient evaluations.

Provide skilled nursing care, including wound care, catheter care, venipuncture, medication management, and patient/family education.

Develop individualized, patient-centered goals and plans of care in collaboration with patients, families, physicians, and other members of the care team.

Coordinate care with physicians and other healthcare providers.

Monitor patient progress and adjust the plan of care as needed.

Educate patients and families about medications, disease processes, treatment plans, and available resources.

Supervise and direct patient care provided by other members of the care team.

Maintain accurate and timely clinical documentation and ensure compliance with applicable regulations. Our EMR is Axxess.

Advocate for your patients and help them remain as independent and comfortable as possible in their own homes.

What We're Looking For

We're looking for someone who is:

Compassionate and genuinely enjoys caring for people.

Organized and detail-oriented.

Self-motivated and able to work independently.

A strong communicator who works well with patients, families, physicians, and coworkers.

Comfortable making clinical decisions and managing competing priorities.

Dependable, professional, and committed to providing high-quality patient care.

Qualifications

Current Utah Registered Nurse (RN) license.

Current Utah driver's license and reliable transportation with required automobile insurance.

Current CPR certification.

Ability to pass a required background check.

At least one year of nursing experience is preferred.

Previous home health or hospice experience is a plus, but not required.

New graduate RNs with a strong interest in home health and hospice are encouraged to apply. Training and support are provided.

Compensation: $36-$45 per hour, based on experience

Typical Schedule: Monday–Friday, 8:30 a.m.–5:00 p.m., with some flexibility.

Join a Team That Makes a Difference

Home health and hospice nursing can be challenging—but it is also incredibly rewarding. You have the opportunity to help patients remain in the place they know and love while supporting their families through important moments in their lives. If you are looking for a nursing position where your clinical skills, compassion, and judgment are valued—and where you can build genuine relationships with patients and families, we’d love to hear from you.
