Job description
Heartland Home Health, part of Gentiva, is seeking a Full-Time Physical Therapist to provide skilled care in patients' homes from our Midland, MI area.
You will evaluate patients, develop goals to improve mobility and independence, collaborate with the care team, educate families, and document progress. A Master's degree in Physical Therapy, Michigan license, CPR, and a valid driver's license are required; a sign-on bonus of $10K and relocation assistance are offered.
