Job description
CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home care under the Clinical Manager. You will plan and administer PT to help patients regain function, with emphasis on safety and quality of life. A $15K sign-on bonus is available for qualified applicants.
Responsibilities include evaluating patients, developing rehab plans, supervising PTAs and home health aides, and documenting progress for physicians and the care team. 40 hours weekly, full-time role.
