Job description
UnitedHealth Group is seeking a Physical Therapist for Home Health within Housecalls Home Health, part of our national post-acute care family. You will assess functional status, develop and implement a treatment plan, and evaluate patient progress to improve well-being in home settings.
Responsibilities include conducting visits, documenting outcomes, and collaborating with care teams to ensure safe, effective care while adhering to state guidelines and patient rights.
