Job description
Acts Retirement-Life Communities is seeking a Home Health Physical Therapist to deliver PT services in residents’ homes, promoting functional independence and safety within a holistic, slower-paced community setting.
Responsibilities include evaluating residents, planning care, and collaborating with an interdisciplinary team to ensure quality outcomes while following state and federal regulations. A Master’s/Doctoral in PT and licensure eligibility are required.
