Job description
Boundless Home Health is seeking a licensed Physical Therapy Assistant to deliver direct therapy services under the supervision of a Physical Therapist. You will implement the Plan of Care, verify orders before each visit, and document changes in patient status to ensure continuity of care.
Responsibilities include performing PTA services per physician orders, adhering to guidelines, and incorporating care goals into daily therapy.
