Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist and Clinical Manager. You will administer treatment plans, perform therapeutic exercises, document records, and participate in in-services to ensure high-quality geriatric care.
The role requires a current PT license, driver's license with reliable transportation, CPR certification, and strong teamwork in a home-health setting.
