Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes in Cottonwood, Arizona. You will implement the Physical Therapist's plan, perform therapeutic exercises, document progress, and coordinate with the care team to ensure high-quality, safe home-based rehabilitation.
Requirements include an active Physical Therapy license, CPR, fingerprint clearance, and a valid driver’s license with reliable transportation.
