Job description
CenterWell Home Health is seeking a dedicated Home Health Physical Therapist Assistant to support patients in their homes under the direction of a Physical Therapist and Clinical Manager. You will implement treatment plans, perform therapeutic exercises, document care, and collaborate with a multidisciplinary team to promote functional recovery and independence.
The role requires mobility to travel to patient homes, current state PT license, CPR, and a valid driver’s license.
