Job description
Optimal Care is seeking a Home Health Physical Therapist to deliver one-on-one rehabilitation services in patients’ homes across Southeast Michigan. You will assess functional abilities, develop personalized plans, and guide interventions to improve mobility, balance, strength, and safety.
