Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes. You will collaborate with a clinical team and report to the Clinical Manager as you deliver personalized care focused on functional improvement.
Responsibilities include planning and administering therapy, assessing patient strengths, developing rehab programs, supervising PTAs and aides, and timely documentation of progress and outcomes.
