Job description
CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home care, helping patients regain strength, mobility, and independence. You will assess, plan, and administer prescribed PT treatments in patients' homes, documenting progress and coordinating with the care team.
Ideal candidates have a current PT license, CPR certification, and experience collaborating with healthcare professionals.
