Job description
NPAworldwide in Greenwich, CT is seeking a Home Health Physical Therapist to provide in-home care, assess mobility, and design personalized exercise plans.
You will collaborate with a clinical team, educate patients and families, and monitor progress to adjust treatment as needed. A CT PT license and at least 1 year of PT experience are required; home health experience is a plus.
