Job description
Compassus is seeking a Home Health Physical Therapist to deliver coordinated care across age groups in a home health setting. You will plan, implement, and evaluate patient care plans, perform PT evaluations, and collaborate with an interdisciplinary team to support patient well-being.
Responsibilities include clinical assessments, developing plans of care with physicians, and providing instruction on transfers and use of assistive devices.
