Job description
CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to travel to patients’ homes, deliver prescribed physical therapy, and help patients regain function. You will assess progress, communicate with the care team, and tailor rehab plans to physician goals and patient needs.
