Job description
Colusa Medical Center is seeking a Home Health Physical Therapist to join the Home Health team in California. The role focuses on providing comprehensive in-home PT assessments, treatments, and caregiver education, under the direction of the Director of Rehabilitation Therapy Services and/or Director of Nursing.
The PT will develop and implement plans of care for adult and geriatric patients, supervise assistants, and contribute to program development while ensuring high-quality, compliant care
