Job description
Valley Health is seeking a Home Health Physical Therapist to provide direct, one-on-one patient care in the home setting as part of a multidisciplinary team. You will supervise assistants, document in an electronic record, and rotate through orientation with a senior preceptor to ensure readiness.
Responsibilities include evaluating patients, designing plans of care, and participating in performance improvement activities.
