Job description
Home Health Advantage INC. is seeking a licensed Physical Therapist to visit patients at home on a part-time, full-time, or per diem basis in Illinois. The role includes implementing home exercise programs, safety evaluations, and fall-prevention strategies to improve strength, range of motion, and muscle education.
You will work with an interdisciplinary team to achieve clinical outcomes and help patients regain mobility in their own homes.
